Referring veterinarians may use this form to request an anesthesiology consultation from CARE.

• A $200 consultation fee applies to all anesthesiology consults. We will follow up with you to process payment once your request is received.
• Responses will come to the email address you provide from CAREAnesthesiology@carecharlotte.com.
• Generally, you can expect a reply within three business days unless otherwise noted.
